This Privacy Policy applies to Go Mad with Nutrition's use of any and all data collected in relation to (1) the use of its website, (2) email correspondence, and (3) the use of services provided by Go Mad with Nutrition.

In order for Go Mad with Nutrition practitioners to provide healthcare services to you, personal information is needed. Our main purpose for collecting, using, holding and sharing your personal information is to manage your health. Your information will also be used for related business activities such as financial claims and payments, practise audits and accreditation.

Personal Information includes information that will identify you as an individual such as;

• Addresses

• Medical information including medical history, medications, supplements, allergies, immunisations, family history, social history and risk factors

There might be several ways of how we collect your personal information.

• Initial Consultation Intake form send prior to your first appointment 
• During the consultation
• Through social media and/or when you visit our website or telephone us
• Via email and/or SMS correspondence 
• Third-party information e.g. referrals or your guardian/responsible person 

When, why and with whom do we share y our personal information? 

• Other healthcare providers 
• When it is required or authorised by law 
• When it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent
• To assist in locating a missing person
• To establish, exercise or defend an equitable claim

Only people who need to access your information will be able to do so. Other than in the course of providing medical services or as otherwise described in this policy, our practice will not share personal information with any third party without your consent.

We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent.

Our practice will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt-out of direct marketing at any time by notifying our practice in writing.

Security, storage and protection of your personal information

Your personal information may be managed and stored in various ways.

• Paper records
• Electronic records - we use a cloud-based management system called NutriAdmin which may be used for the storage of your personal information. If you would like further information regarding NutriAdmin's privacy policy, please email the clinic at gomadwithnutrition@gmail.com
• Audio 

All personal information is stored securely. Electronic files are stored using password protected proprietary within NutriAdmin. All paper hard copy files are stored in lockable filing cabinets.  

How you can access and correct your Personal Information

You have the right to request access to your Personal Information as well as correcting these. 

If you do wish to request access to your Personal Information please do so in writing by emailing the clinic at gomadwithnutrition@gmail.com. This process might take up to 30 days. No fees will be charged for your request, but an administrative fee might apply for any costs complying with the request. 

In order to ensure that your Personal Information is accurate or up to date, we might request verification of your Personal Information from time to time. You also can request to update your Personal Information. This should be done either in writing or during a consultation with your nutritionist.
